Obtaining a Passport

IF YOU HAVE NEVER HAD A PASSPORT


You will need:

  1. A certified copy of your Birth Certificate - this MUST have a State or County raised seal and must include the date it was filed. Naturalize citizens are required to submit their Certificate of Naturalization.
  2. Two passport pictures (2" x 2").
  3. Your Driver's License or State ID.
  4. A check or money order payable to Department of State for (see fee info below).
  5. A check or money order payable to the Ionia County Clerk for $30

 

OTHER INFORMATION YOU ARE REQUIRED TO KNOW

  1. Your social security number
  2. Your father's birthplace and birth date
  3. Your mother's maiden name, birthplace and birth date
  4. Date of most recent marriage
  5. Date when widowed/divorced - if applicable
  6. Naturalization information - if applicable



IF YOU HAVE HAD A PASSPORT BEFORE AND ARE ELIGIBLE TO RENEW BY MAIL
(forms available in the Clerk's office or downloadable here).

You will need:

  1. Your old passport (if you do not have this anymore, you will have to fill out a DSP-64)
  2. Two passport pictures (2" x 2").
  3. A check or money order payable to DEPARTMENT OF STATE for $40.00.


FEES:


Age 16 years and over (Passport is good for 10 years):
$45.00 Check of Money Order payable to the DEPARTMENT OF STATE.
$15.00 for Ionia County Clerk


Age 15 years and under (Passport is good for 5 years):
$25.00 Check or Money Order payable to the DEPARTMENT OF STATE
$15.00 for Ionia County Clerk


Children 13 years and older must sign their own applications. If applicant is under 13 years, one of their parents MUST sign their application.
