Reservation Forms

These forms can be printed, filled out, and mailed to the Bertha Brock Park office at 2311 W. Bluewater Hwy., Ionia, MI 48846.  You may also obtain a copy at the park office.  Prior to submitting a reservation request, please call the Bertha Brock Park office at 616.527.7275 to check availability.

Cabin Reservation Form
This form is for reserving camping cabins at Bertha Brock Park only. 

General Reservation Form
This form is for general reservations such as picnic sites, pavilions, the Palmer Lodge, warming sheds, stone bridges, the ball field, etc.
